Dear Applicant,

Enclosed please find our service dog application packet. Only those applications which are returned complete (all information included) will be processed, so please read the following instructions carefully. If you have any questions, do not hesitate to call for clarification.

A completed application includes the following:

1. A $25 application fee.

2. The completed Program Application form.

3. The Medical History form completed by your physician or primary care specialist.

4. A personal letter of reference from a friend, teacher, or someone other than a family member.

5. A professional letter of reference from a therapist, social worker, teacher or any other professional with whom you have contact.

Your completed application will be reviewed by staff upon receipt. Those applicants who qualify for our program will be sent a preliminary interview form. Upon receipt of the pre-interview form, either a personal or phone interview will be scheduled.

The cost of a service dog is $3000, and the two week training camp is $500. Additionally, expenses during the training camp are the responsibility of the client. If you need financial assistance, contact the Assistance Dog United Campaign at 1-800-284-DOGS to see if you qualify for a voucher.
