Dear Applicant,

Enclosed please find the application you requested. Only those applications which are returned complete (all information included) will be processed, so please read the following instructions carefully. If you have any questions, do not hesitate to call for clarification.

A completed application includes the following:

1. A $25 application fee.

2. The completed Program Application form.

3. A written description of how the dog will be used in your setting or with you, how the dog care and handling will be taken care of (for example, who will feed, groom, toilet the dog), who will be the dog’s primary caretaker and will the dog go home on weekends and evenings with this person, and how many hours per day the dog will be working.

4. A personal or professional letter of reference.

Your completed application will be reviewed by staff upon receipt. Those applicants who qualify for our program will have either a phone or personal interview and then be put on the waiting list to receive a dog.

The cost of a therapy dog is $1000, and the one week training camp is $250. Additionally, expenses during the training camp are the responsibility of the client.
